Guidelines for completing the Online Application Form

Before you begin filling out the Online Application Form, please note the following important information:

  1. Eligibility: Completing the Online Application Form does not automatically qualify you for the Ghana Card. Eligibility will be determined after the full application and interview process.
  2. Registration Process: Submitting the Online Application Form does not complete the registration process. You will be required to attend an interview to finalize your registration.
  3. Language: The interview(s) following your application will be conducted in English. If you require assistance, please arrange for a qualified interpreter to accompany you to your interview appointment.
  4. Guidance: Detailed instructions are provided to assist you in completing the form. If you're unsure about how to fill in any section, leave it blank. An interviewer will guide you during your appointment.
  5. Required Documents: During your interview and biometric capture, you must present the original copies of all mandatory and supporting documents. The NIA will not accept photocopies of these documents.
  6. Acceptable Documents: Only the following documents are considered mandatory for registration for the Ghana Card:
    • A valid Ghana Passport
    • A Birth Certificate issued by the Births and Deaths Registry, Ghana
    • A Certificate of Acquired Citizenship
    • A Naturalization Document
  7. Please note that other Government-issued ID cards and documents do not qualify as mandatory documents for confirming eligibility to apply for the Ghana Card.

  8. Digital Address Code: You are required to state the digital address code of your residence in Ghana. This is not required if you live outside Ghana.
  9. Submission and Fees: After completing the Online Application Form, ensure you submit it electronically, pay the required fees, and book an appointment for your interview(s).
  10. NOTE: The Ghana Card application fee is non-refundable, regardless of the stage or the outcome of your application.

  11. Appointments: You may reschedule your in-person appointment up to three (3) times following your initial booking. Please note that if you do not reschedule before the specified date and time, you will forfeit the opportunity to reschedule again and will not be eligible for a refund on any payments made.
  12. Vouching Requirements: If you do not have any of the mandatory documents, you may apply for the Ghana Card using the Vouching Process. Any individual vouching for you as a Ghanaian must:
    • Be at least 18 years old
    • Be of sound mind
    • Hold a valid Ghana Card
    • Be legally permitted to vouch for applicants under the law.
  13. The following persons may vouch for you:
    a. One (1) relative aged 18 years or older who possesses a Ghana Card. The list includes:
    • Father or Stepfather
    • Mother or Stepmother
    • Uncle or Aunt
    • Brother or Half-Brother
    • Sister or Half-Sister
    • Nephew or Niece
    • Grandfather or Grandmother
    • Son or Daughter
    • Cousin
    • Husband or Wife
    • Father-in-Law or Mother-in-Law
    • Brother-in-Law or Sister-in-Law
    • Son-in-Law or Daughter-in-Law
    • Or:
      a. Two (2) people aged 18 years or older, who possess a Ghana Card. The two people authorised to vouch for you are as follows:
    • A practicing or retired professional teacher (including principals, headmasters, and headteachers)
    • A Gazetted Chief
    • A practicing or retired Magistrate or Judge
    • A practicing or retired licensed professional (e.g., doctor, nurse, lawyer, accountant, engineer, architect)
    • A serving or retired Civil Servant or Public Servant
    • A serving or retired Clergyman
    • An Imam
    • A Catechist
    • A serving or retired member of the security services (e.g., Ghana Police Service, Ghana Armed Forces, Ghana Prisons Service, Ghana Immigration Service, Ghana National Fire Service, National Intelligence Bureau, Customs, Excise, and Preventive Service, National Security)
    • The President or Vice-President or Secretary of a Ghanaian national, cultural, professional or trade association in a foreign country duly registered with the Ghana Mission of that country (applicable to Ghanaians living outside Ghana only)
    • A current or past Member of Parliament
    • A current or past Assembly or Unit Committee Member
  14. Additional Government-Issued ID Cards and Documents: If you have any of the following documents, you are required to state them and bring them along for data harmonization and integration purposes only:
    • Voter ID Card
    • SSNIT ID Card
    • Driver’s License
    • National Health Insurance Card
    • Tax Identification Number (TIN)
